2018-2019 Registration Form

Registration fee ($30 for 1 child and $40 for 2 or more) is due upon registration.  Open Enrollment for non-YEP parents begins May 1, 2018.

We are now accepting Debit, Visa, Mastercard and Discover cards as forms of payments in the YEP office and online.  There is a convenience fee of 2.35% per transaction.  

Additions to Pick-Up Form

The person who registers the child(ren) is the only person who can add or remove persons from the pick-up list.

Any changes need to be submitted 24 hours prior to the new person picking-up the child(ren).

Address and Phone Number Change Form

The person who registers the child(ren) is the only person who can make any changes.

To update any contact information (address/home, cell, and/or work numbers) use this form.

Drop Form

The person who registers the child(ren) is the only person who can make any changes.

In order to drop a child(ren) from the program, this form is needed and may be faxed back to the office at 318-865-2237.

Please give two weeks notice.

Status Change Form

The person who registered the child(ren) is the only person who can make status changes.

Please indicate the type of change (part-time to full-time, full-time to part-time) you are requesting in the corresponding section.
