How to Register for YEP

Follow these three simple steps to secure your child’s spot in our program.

1. Prepare Your Documents

• Find the Form: Go to the “Documents” section on our website to download and print the YEP registration form.
• Complete the Form: Fill out all required fields for each child you are enrolling.
• Gather Records: Obtain a copy of your child(ren)’s immunization records; these must be submitted with your form.

2. Submit Your Application

We prefer submissions in PDF format. Please avoid sending photos or faxes. If you are registering more than one child, include all forms in a single submission.

• Via Email (Preferred): Send to Enrollment@yep-la.org.
• Via Mail: 4700 Line Avenue, Suite 207, Shreveport, LA 71106.

3. Confirm & Pay

Once we receive your forms, watch for two emails:

1. Welcome Email: Confirmation from our Enrollment Coordinator.
2. Invoice: A digital invoice from our Bookkeeper. Simply click the blue “Review and Pay” button to pay online.
   • Note: If a site is full, you will be notified and added to the waiting list.

We accept credit/debit cards, checks*, and money orders*.

*If paying by check/money order, please include your child’s name and school on the payment.
