2019-2020 Registration Form                                                                          

Registration fee ($30 for 1 child and $40 for 2 or more) is due upon registration.  Open Enrollment for non-YEP parents begins May 1, 2018. New YEP parents will need to submit the registration form, current immunization records, and registration fee.

If registering your child(ren) for the first time, you must make your initial payment in the office.  You can not make online payments until your first payment has been made in the office. 

We are now accepting Debit, Visa, MasterCard and Discover cards as forms of payments in the YEP office and online.  There is a convenience fee of 2.35% per transaction.  YEP does not accept American Express as a form of payment.  If American Express is used, you will be financially responsible for all additional charges incurred. 

When paying fees online use the payment schedule below:

Additions to Pick-Up Form

The person who registers the child(ren) is the only person who can add or remove persons from the pick-up list.

Any changes need to be submitted 24 hours prior to the new person picking-up the child(ren).

This form may be faxed back to the office at 318-865-2237.

Address and Phone Number Change Form

The person who registers the child(ren) is the only person who can make any changes.

To update any contact information (address/home, cell, and/or work numbers) use this form.

This form may be faxed back to the office at 318-865-2237.

Drop Form

The person who registers the child(ren) is the only person who can make any changes.

In order to drop a child(ren) from the program, this form is needed and may be faxed back to the office at 318-865-2237.

Please give two weeks notice.

Status Change Form

The person who registered the child(ren) is the only person who can make status changes.

Please indicate the type of change (part-time to full-time, full-time to part-time) you are requesting in the corresponding section.
