To submit your child(ren)’s registration form, please email to Enrollment@yep-la.org.  

DO NOT FAX REGISTRATION FORMS. 

Please read the registration form thoroughly and initial each line on pages 3 and 4 (no checks or x’s)

YEP does not administer medication.

We accept check and money orders in the YEP office and debit/credit cards as forms of payment online.  There is a convenience fee of 2.35% per debit/credit card transaction.  

When paying fees online use the payment schedule below:

                 2 children     $321.38                    $255.88

                 3 children     $470.95                    $378.70

Additions to Pick-Up Form

The Account Owner, the person who signs the registration form(s), is the only person who can add or remove persons from the pick-up list.

Any changes need to be submitted 24 hours prior to the new person picking-up the child(ren).

This form may be faxed back to the office at 318-865-2237.

Address and Phone Number Change Form

The Account Owner, the person who signs the registration form(s), is the only person who can make any changes.

To update any contact information (address/home, cell, and/or work numbers) use this form.

This form may be faxed back to the office at 318-865-2237.

Drop Form

The Account Owner, the person who signs the registration form(s), is the only person who can make any changes.

In order to drop a child(ren) from the program, this form is needed and may be faxed back to the office at 318-865-2237.

Please give two weeks notice.

Status Change Form

The Account Owner, the person who signs the registration form(s), is the only person who can make status changes.

Please indicate the type of change (part-time to full-time, full-time to part-time) you are requesting in the corresponding section.
